When a job offer is made
- You will receive a formal offer letter from the Human Resources (HR) division.
- Review, sign and return your offer letter immediately to the HR contact designated in the letter.
- Your HR representative will schedule you for new hire orientation.
- Review all new hire forms listed below. Pay special attention to the document's explanation to make sure you understand how to complete each form based on your individual circumstance.
Note: All forms will be provided at new hire orientation and you will be briefed on how to complete them.

Form I-9 U.S. Citizen Acceptable Document List (pdf) Required for New Employee Orientation |
You are required to provide original, UNEXPIRED documentation from the list of acceptable documents. Employees are required to present one selection from "List A" (no other document is needed), OR one selection from "List B" in combination with one selection from "List C." Original documentation must be provided. Important: Expired, laminated, electronic documents and/or hospital birth certificates are NOT acceptable documents. Failure to provide required documents within three business days could result in termination of employment. |

You are required to provide original, unexpired documentation from the list of acceptable documents. Employees are required to present one selection from "List A" (no other document is needed), OR one selection from "List B" in combination with one selection from "List C." Original documentation must be provided. In addition, depending on your visa status, you must provide documentation of your current work authorization. Examples: J-1 will present DS-2019 Certificate of Eligibility for Exchange Visitor Status; F-1 will present I-20 Certificate of Eligibility for Nonimmigrant; and/or a current I-94 Form. You MUST provide a printed I-94. Expired documents are NOT acceptable documents. Failure to provide required documents within three business days could result in termination of employment. |
