Required Forms

Documents you will sign on the first day

For more efficient orientation, all forms may be filled out prior to arrival with the exception of the I-9 form.

• Employee Profile Form (pdf)
• W-4 Form: Complete Form W-4 so that your employer can withhold the correct Federal income tax from your pay. Because your tax situation may change, you may want to refigure your withholding each year.
• Laboratory policy regarding government property (pdf)
• List of Acceptable Documents (pdf) (for I.D. and Employment Eligibility (I-9): Required documents at the time of hire.
• Payroll Direct Deposit (pdf): This form will be submitted to payroll to allow for direct payroll deposits to your checking and/or savings accounts.
