LANS Core Employee (core package)
Before you begin the Introduction presentation, please print out the "Medical Plan Enrollment Form" and "Medical Plan Monthly Rates" documents. You will need to complete the medical plan form as you view the presentations for each benefit. After you have completed all presentation segments we encourage you to view the supporting documents to provide further assistance in making enrollment decisions.
Please note: as of January 1, 2009 all premiums are deducted on a semi-monthly basis.
