Life Insurance and Disability Options

Although your life and disability benefits are not options that you may modify through the ORACLE Self-Service application during open enrollment, you are eligible to apply for changes to these benefits at any time during the year by completing the statement of health process through our insurance carrier, The Hartford.

Basic Life (Employer Paid)

The Laboratory pays premiums for basic life insurance coverage in full. The principal is 1 x your annual salary, up to a maximum of $50,000.

Supplemental Life (Employee Paid)

If the basic life option does not provide enough financial security, Laboratory employees may choose to purchase their own supplemental life policy through the LANS group plan. Premiums are paid through after-tax payroll deduction. You may choose the amount of coverage, from 1 x to 5 x your salary, up to a maximum coverage amount of $1 million.

Employees who want to increase their coverage to a higher level of benefit at any time during the year must submit a statement of health for approval by The Hartford and then contact the Benefits Office within 31 days of the letter of approval.

Dependent Life (Employee Paid)

Employees may also choose to insure their eligible dependents. There are both basic and expanded dependent life plans available, and you may choose to insure only your spouse/same-sex domestic partner, only your eligible children, or all of you eligible family members. Any time a spouse/same-sex domestic partner is enrolled with a covered benefit of $50,000 or greater, statement of health through The Hartford is required.

Short Term Disability (Employer Paid)

Laboratory employees are automatically enrolled in a short-term disability plan that provides basic short-term wage replacement for non-work related disabilities, including pregnancy, disabling injury or illness. This coverage pays 55 percent of your eligible earnings, up to $800/month, for a maximum of 26 weeks. You must be under a doctor’s direct and continuous care. The Laboratory pays the entire premium cost for your short-term disability coverage.

Supplemental Disability Insurance

Laboratory employees may purchase additional disability insurance, which pays a higher level of benefit for a longer period of time, if you are unable to work due to non-work related disabilities, including pregnancy, disabling injury or illness. Cost will depend on the waiting period you choose, your age as of January 1 of the current year, and your salary rate as of January 1 of the current year.

Benefits will commence after meeting your chosen waiting period (7, 30, 90 or 180 days) or after you exhaust your required sick leave, whichever occurs later. As with short-term disability, you must be under a doctor’s direct and continuous care.

Accidental Death & Dismemberment (AD&D)

There will be no rate increase or plan design changes for AD&D in 2010. AD&D insurance protects you and your family from the unforeseen financial hardship of an accident that causes death, dismemberment, or loss of sight, speech, or hearing. The plan provides worldwide coverage for you and your enrolled family members.

Business Travel Accident (Employer Paid)

There are no plan design changes to the business travel accident plan in 2010. Business travel accident insurance covers accidental death or dismemberment of Laboratory employees traveling on official Laboratory business or while engaged in designated hazardous activities on behalf of the Laboratory.

If you are eligible, you will be covered 24 hours/day, worldwide, up to $100,000 or, if you are a senior manager, $250,000. This coverage is in addition to other insurance you may have at the time of the accident.
