Reporting a Death

When the death of a LANS employee or dependent occurs, you must:

Step 1 Notify the Lab's Benefits Office at 505-667-1806. Upon notification, a Benefits Representative will begin the claim process.
Step 2 Complete all the paperwork that will be sent to you by mail from LANS.
Step 3 Provide official copies of the death certificate to the Benefits Representative assigned to your case.
Step 4 Inform the Benefits Representative assigned to your case about any issues associated with the claim.

You are also required to de-enroll a deceased family member from all insurance plans. Contact the Lab’s Benefits Office for assistance.
