When the death of a LANS employee or dependent occurs, you must:
| Step 1 | Notify the Lab's Benefits Office at 505-667-1806. Upon notification, a Benefits Representative will begin the claim process. |

| Step 2 | Complete all the paperwork that will be sent to you by mail from LANS. |
| Step 3 | Provide official copies of the death certificate to the Benefits Representative assigned to your case. |
| Step 4 | Inform the Benefits Representative assigned to your case about any issues associated with the claim. |
You are also required to de-enroll a deceased family member from all insurance plans. Contact the Lab’s Benefits Office for assistance.
