Thursday, April 8, 2004 Notifications and ISM-based investigations of safety events at the LabThis notice provides requirements that must be implemented for making notifications and conducting Integrated Safety Management-based investigations of safety events at the Laboratory. After a series of events at the Lab that included a chlorine dioxide explosion and an electrical shock, Laboratory Director G. Peter Nanos issued a memorandum (DIR-03-071) that required an improved and more timely method of ISM-based investigation. This notice provides the requirements that must be implemented to make notifications and conduct the following types of investigations: (1) team investigations of high-significance events reported to the Lab director; (2) investigations of medium-significance events reported through the Occurrence Reporting and Processing System; and (3) group-level investigations of minimal-significance safety events recorded in the Management Walkaround system.
