Exclusive Provider Organization

Consumer‐Directed Health Plan
(CDHP) medical plan
- Available only to retirees who are NOT coordinated with Medicare
- Laboratory contributes one-half of your total deductible into a Health Reimbursement Account (HRA). Until the HRA is exhausted, you pay nothing out of pocket. Once HRA is exhausted, you pay remainder of the deductible. After deductible has been met, you pay 10% co-insurance for eligible in-network services. If HRA is not exhausted, unused money roles over to the next plan year.

National Medicare Supplement
- Available only to retirees who are coordinated with Medicare Parts A & B.
- Complements Medicare by paying specific benefits for Medicare-eligible medical services. You must go to a provider who has accepted Medicare assignment.
